Help with a Google Docs Query


NOTE:  The names are all invented and do not represent real student information.

The link above is for a spreadsheet problem I would like help with.  The main sheet is called 'Form Responses' which is populated when teachers use Google forms to note when a student leaves their classrooms.  I was very appreciative of some help I got last week and now have sheets (not shown here) that pull data from Form Responses for each grade.  I now want to able to do a similar summary for the destinations.  For, for example, the guidance counselors will be able to consult a sheet that looks like this:


Filtering for rows in which D contains "Guidance," I basically want to pull Timestamp, Username, Destination, and Notes directly from 'Form Responses."  I want to populate Student with data from 8th Grade Students or 7th Grade Students or 6th Grade Students or 5th Grade Students.

Any help would be greatly appreciated.

In the following solution I used VMERGE custom written function by ahab available via Script gallery from within the spreadsheet to vertically merge the data for 8th, 7th, 6th, and 5th grade students and then queried the data to meet the specified criteria
