I am trying to figure out a formula or sequence of combined formulas that can accomplish a populated date based on a series of values.

Example #1: If blood was donated on 3/11/2015 and 450 mls of blood was taken then the desired date that needs to be populated will be 4 weeks (28 days) from 3/11/2015. (answer needs to be 4/9/2015)

Example #2: If blood was donated on 3/11/2015 and 1000mls of blood was taken then the desired date that needs to be populated would be 8 weeks (56 days) from 3/11/2015. (answer needs to be 5/7/2015)

The number value (450 or 1000) needs to determine how many days (28 days or 56 days) to count to the desired calendar date (answer), using the original date (date blood was donated) as the starting point .

450 and 1000 would be the only ones used for now (unless we wanted to add any others later. The quantity of blood taken is dependent on body weight). The way we have it set up is through a form that imports into a sheet.

The first sheet (sheet 1) will collect the mls quantity of blood (450 or 100), and the date the blood was taken and then on a second worksheet (sheet 2) We want it to list the donor's name and the date they can donate again (have to wait 4 or 8 weeks depending on how much blood was taken last.)
