For example, the Key in this Form is the "Type of Call" field where the user can input AM, PM or Mid-Day and will be asked 1 question.
What I would like to create is a query that summarizes, by "Facility" or user, the 3 questions asked into 1 line but only for the most current day.
So...if an array is set up like:
Date Facility Type of Call Question 1 Question 2 Question 3
