Google Query Help Summarizing Google Form
I have created a Google Form - made public for anyone's review - that depending on the selection criteria will ask certain questions and fill in new rows for each entry. For example, the Key in this Form is the "Type of Call" field where the user can input AM, PM or Mid-Day and will be asked 1 question.

What I would like to create is a query that summarizes, by "Facility" or user, the 3 questions asked into 1 line but only for the most current day.
So...if an array is set up like:
Date Facility Type of Call Question 1 Question 2 Question 3
12/10 Mike AM Yes
12/10 Mike Mid-Day No
12/10 Mike PM Yes
12/14 Jim AM No
12/14 Jim PM No
I would like the query's result to be:
12/10 Mike Yes No Yes
12/14 Jim No No
